Greater Family Health is a joint commission-accredited, primary care medical home-certified federally qualified Health center providing comprehensive access to high-quality, affordable and integrated Health care in the specialties of Family practice, pediatrics, obstetrics & gynecology, oral Health and behavioral Health. Through 11 Health centers located in the communities of dekalb, elgin, franklin park, hanover park, mchenry, palatine, streamwood, sycamore and wheeling. Greater Family Health was able to provide care to 67,963 patients through 214,604 visits during the 12-month period ending june 30, 2023. Furthermore, Greater Family Health has again been recognized by the us department of Health and human services, Health resources and services administration with 5 national awards including, national quality leader for the 10th consecutive year, with our cy 2022 clinical outcomes ranked in the top 10% nationally.
