Program areas at CMCS

Opwdd programs:1) Services for individuals with developmental disabilities - these include 18 individual residential group homes for more than 100 adults with developmental disabilities. These 100 residents are provided with exceptional care and support, 24 hours a day, and are staffed by support professionals committed to providing opportunities for personal growth and development; day Services for 57 adults that focuses on vocational skill building and Community integration and activities that improve socialization skills. 2) the opwdd family support Services opwdd family support Services funds short term family assistance for families in westchester county that do not currently have medicaid. A service coordinator assists the families with completing subsidy applications, such as social security, medicaid, food stamps or other assistance. The service coordinator also works with the family in locating Services for the individual in their family with a developmental disability. The Services may include a day and/or residential program, recreation and respite Services. The service coordination Services are provided until the individual is eligible and enrolled to receive medicaid benefits and can then receive through our medicaid service coordination.
Family and group day care:family day care provides in-home day care Services and a nutritional food program for 353 Children each day and supports approximately 120 day care providers with training and supervision. The program seeks to strengthen family life and maintain Children in a healthy, safe and educational environment with nutritious food and multiple creative techniques for development and success. We are one of the largest family child networks in nyc.
Early childhood education: our early childhood education division (eced) serves over 1035 Children and families in the bronx. The programs focus on education along with offering a wide range of support including health, nutrition, mental health, medical, and family engagement and involvement. The cmcs eced division provides Services under three program groups; head start, in conjunction with the federal hhs administration for Children and families; the state funded universal pre-kindergarten program in partnership with the nyc department of education; and early learn new york city (elnyc), including center based full day care.
Health and clinical services:our health and clinical service programs provide coordination and oversight of medical care and behavioral health care for Children in foster care, group homes, hayden house emergency group residence and individuals with developmental disabilities living in group residences and participating in day habilitation. More than 225 Children and adults received health and clinical Services.
Special medical and developmental preventive Services and family rehabilitation programs:special medical and developmental and family rehabilitation preventive Services programs provide families who are at risk of having their Children placed in foster care with intensive family support, outreach and intervention Services to address the risks to child well-being and safety. Approximately 244 families with 548 Children in the bronx and harlem received these Services.
Hayden house emergency group residence and school:hayden house emergency group residence and School provides emergency "safe house" Services for abused and neglected Children in westchester county. It provides residential care, diagnostic evaluation, trauma-informed counseling and placement planning. Hayden provides around-the-clock emergency intervention to Children in crisis between the ages of 8 and 18. Once at hayden house, Children receive temporary care until suitable and safe living arrangements are made. During this time each child receives a total diagnostic evaluation and a comprehensive care plan so they can begin to put their lives back together. The School, located at hayden house, serves grades 4 through 12 following the nys-mandated regents curriculum. Children are required to take and pass all standardized tests following the nys department of education testing schedule. We provide students with academic support, personalized intervention and remedial education through aspire, our after-school program that uses computers to help the Children catch up academically. During this year we did not served Children at hayden house.
Runaway and homeless youth sunrise drop-in center:located in the south bronx, this program serves vulnerable and "at risk" youth, including runaway and homeless youth. The center provides a haven in which a teen's basic needs are met and can obtain assistance with engaging in education, employment, mental health Services and case management Services. The drop-in center also serves as a resource to assist youth in obtaining shelter and temporary and permanent housing. Approximately 344 youth were served at the drop-in center.
Health Home care management:in 2016 cmcs implemented a care management program for Children, operated in conjunction with the collaborative for Children and families health Home. Care management is a voluntary service for medicaid-eligible Children who have who have qualifying medical, developmental or emotional diagnoses and whose families need support in accessing and navigating the healthcare system to meet their child's medical and behavioral health needs. Care managers work closely with caretakers and Children in the program to identify and assess their needs, coordinate a plan of care and integrate Services across various treatment and other providers, with the goal of ensuring access to all of the Services the child needs for well-being and healthy growth and development. During fy 2022 443 Children were enrolled in the program.
Mccloskey care clinic:in january 2019, cmcs took a major step forward in our commitment to child and family well-being. We opened the Mccloskey cares clinic, a new mental health clinic located at cmcs' main service facility in the south bronx. The Mccloskey cares clinic is a natural extension of our mission to provide a continuum of community-based Services to vulnerable Children, families and adults using a trauma-informed, holistic, and family-centric approach. Our clinic is designed for at-risk Children and youth ages 3-24 years and their families, and primarily serves communities with high levels of poverty, including mott haven and hunts point where we already maintain a significant program presence. Our clients reflect the population of the south bronxmost are low-income, hispanic/latino and african-american, with a high percentage of immigrants. The clinic is also positioned to work with clients who are known to our agency through other programs we operate, including Children and youth in our foster care and family support programs, runaway and homeless youth in our drop-in center, and Children in our health Home care management program who have behavioral health diagnoses and present with complex risk factors. Approximately 199 Children were served at the clinic.
Child and family treatment and support Services and Home and Community based services:cmcs offers community-based Services and support for Children and families, under the auspices of two recently restructured nys medicaid waiver programs: child and family treatment and support Services (cftss) and Home and Community based Services (hcbs). Services are offered to Children and families in nontraditional settings, such as the child's Home or Community locations, in order to provide a flexible approach to service provision that facilitates engagement and/or therapeutic benefit. Approximately 175 individuals were served at the clinic.
Family foster care/treatment family foster care: family foster care/treatment family foster care provides family-based care, case management Services and access to needed treatments and support, and permanency planning Services, including reunification, kin-gap and adoption to Children removed from their homes due to abuse or neglect. Family foster care also supports certified foster parents with training and supervision. Treatment family foster care is designed for Children with more serious emotional disturbances and other mental health and behavioral conditions that call for specially trained foster parents and a more intensive set of Services. Approximately 260 Children and their families were served in in our foster care programs and 7 Children were adopted.
