Medical hiv services:can Community Health, Inc. medical services program is designed to provide a continuum of care and medical treatment services to individuals and families living with and affected by hiv/aids, hepatitis c, sexually transmitted diseases and other diseases. Patients have encounters with physicians, case managers, nursing staff, and specialty medical providers who assess and design an interdisciplinary plan of care, regardless of their ability to pay. Additionally, our education team assists patients to be adherent with their medical regime. Via a statewide outreach effort, the team provides free, confidential and anonymous hiv and std testing, counseling, and resource identification (internal resources as well as available with various partner organization) to the Community. Can Community Health, Inc. Served 17,290 clients through multiple centers located throughout the states of Florida, Arizona, Virginia, new jersey, and south carolina.
Pharmacy assistance program:can Community Health participates in the federal 340b drug discount program. Under the 340b program, drug manufacturers provide medications at a discounted rate to approved safety net healthcare providers (such as Can Community Health) for patients who receive medical services from these safety net healthcare providers. Enacted in 1992 and administered by the office of pharmacy affairs, the federal program (which does not rely on tax dollars) is designed to help covered entities "stretch scarce federal resources as far as possible, reaching more eligible patients and providing more comprehensive services". Can Community Health generates savings by procuring drugs at a discounted rate for our eligible patients. These savings allow us to fund and create ways to broaden healthcare related services, produce better patient outcomes, support a variety of client / patient assistance programs, and increase the scope of services that are made available to low-income, uninsured and underinsured patients throughout the communities we serve. Our 340b pharmacy program continues to grow with patients now participating throughout our statewide network of clinics, providers, and pharmacies.
Dental hiv services:can Community Health's dental services program is designed to provide treatment and services to those living with and affected by hiv and std diseases regardless of the ability to pay. Dental problems that are left unaddressed to a person with hiv may lead to a host of other medical issues. Accordingly, our dental program provides services such as cleanings, dental evaluations and exams, extractions, dentures, oral cancer screenings, x-rays, as well as several other services that fit to meet the needs of the patients. These dental services are an important part of the success in the overall plan of care for an individual living with hiv. Education is reinforced at each dental visit as well as at periodic case management and medical visits. This past year over 1,758 dental visits were made to the organization.
Case management services:can Community Health's case management team provides service coordination, screening and enrollment assistance with federal, state, and local programs that benefit people with hiv, std's, and other diseases. In addition to our in-house client assistance resources, other programs and grants supported include hip (high impact prevention), hopwa (housing opportunities persons living with aids), ryan white, to name a few.
