Rosemount Basic Health Major Medical and Dental Plan Trust

The organization is a reporting welfare benefit Plan (Plan) for the benefit of employees of Rosemount, Inc., its subsidiaries and affiliates. Under the terms of Trust agreements between u.s. bank national association (trustee) and the Plan, the trustee manages the Plan's Trust funds on behalf of the Plan. The Plan administrator maintains the exclusive right to control and manage the Plan assets. Significant changes to the Plan may be required annually by virture of changes in federal law or the regulations applicable thereto.
