bridge housing - a- greater self-determination - goal: 80% of families will accomplish no less than two goals in their individual case plans (including mental health therapy goals)- outcome: in fy 2020-2021 we had 81% of our families accomplish no less than two goals in their individual case plans. This includes saving money/budget, decreasing the intensity of depression and finding better jobs to increase their incomeb- increased skills or income- 1- goal: 90% of families will receive entitlement services that they qualify for from other community agencies- 2- goal: 70 % of all families will achieve and maintain employment and either maintain or increase their total income (from all sources) upon successful completion of the program- outcome: 100% of families who were eligible to receive social services, were referred and given i.e. CalFresh, cash aid, and state health insurance coverage; medical. - outcome: 90% of families maintained employment and 45% had an increase of income from jobsc- residential stability- goal: 70% of families will graduate to permanent housing (subsidized or non-subsidized) upon successfully completing the bridge housing program - outcome: there were 85% of families who graduated to permanent housing (subsidized or non-subsidized) after successfully completing the bridge housing program. they moved to apartments and mobile homes, both in state and out of state. many of our families found that moving out of state afforded them a larger home for less money. They also had jobs waiting for them out of state. Summary - the adults in our family were able to maintain or find jobs during fy 2020-2021. a few families were able to work remotely from home. this assisted single parents in helping their children with school schedules as all of the schools had remote learning in 2020-2021. we were so proud of our parents balancing the best they could their children's school schedules and their need to work. we did have a few adults who worked more than one job. those adults who worked two jobs, the majority were two-parent families, though we did have single mothers who also had two remote jobs this past fiscal year.
