Program areas at First Community Health Foundation
First Community Health Foundation (fchf) promotes Health and disease prevention and actively participates in efforts to identify Community needs and opportunities in dealing with chronic disease issues as well as finding a means to overcome them.fchf does business as "the victory fund" for fund raising and marketing purposes. It is the public name and face of the Foundation and is used exclusively. It is important for the Foundation to present a strong name to reflect the commitment it has to fighting and becoming victorious over chronic disease in the communities it serves. The fund is working to promote Health education, Health literacy, overall mind and body wellness, and access to innovative and affordable Health care practices.
The victory fund awarded grants to several organizations that promote healthy eating through education to k-5 grade students, farm to school lunch programs, and the development of Community gardens in low-income neighborhoods in "food desserts." By funding these programs, the victory fund supports an "up stream" approach to eliminating chronic diseases exacerbated by poor eating habits and lack of healthy food options and education. Grants given include: voyageurs k-5 program - $100,000 spread evenly between 2015 to 2017; duluth Community garden program - $28,000; generations Health care initiatives - bridge to Health survey - $10,000; Community Health board to provide schools training in providing healthy meals for kids - $34,000; and a $70,000 grant to life house, an organization that provides housing programs for homeless, at-risk youth, was made specifically to develop a program addressing chronic mental Health needs.
Legacy awards and programs: the victory fund continues to sponsor the legacy organization's scholarship funds - the founders' fund and the sustainers fund. The award is $1,000 paid over two years. Two to three awards are made each year. In 2015, the Foundation paid $4,000 from these funds. The victory fund awarded $322,675 to lake county ambulance service to honor the legacy of their organization. The fully-equipped, state-of-the-art als ambulance will allow the service to expand their service area and reach more individuals in crisis in rural, hard-to-reach areas. $1,000 was also awarded for miscellaneous, mission-related donations.
Discontinued operations - firstsolutions ceased quoting and writing insured policies as a Health maintenance organization (hmo) in 2009. Discontinued operations on the financial statements include amounts related to the cessation of this operation. The gross revenue includes final settlements on premiums and claims.firstsolutions, by board action, sold its clinics and pharmacies as of april 1, 2011. Discontinued operations on the 2012 financial statements also include amounts related to the recovery of Health care receivables.
The victory chorusthe victory chorus supports the initiative of becoming a dementia friendly community.the chorus offers the residents of duluth who have been affected by dementia with a unique opportunity to build connections, seek joy and meaningful engagement in their Community through song. The chorus provides a venue for caregivers and persons with dementia an opportunity to have a shared experience which promotes their strengths and abilities despite disease. Through participation in rehearsals and public performances, the chorus promotes self-discovery, learning, laughter and a sense of inclusion.
Dementia friendly duluth (dfd)our goal is to make the Community more aware, informed, sensitive and supportive of people experiencing and caring for those with alzheimer's and other forms of dementia.our objective is to improve awareness of the resources and organizations available to support people with dementia and their families and increase knowledge of dementia across the community.dfd established and facilitates dementia collaborative meetings and activities. The collaboration was formed as a program of dfd for the purpose of networking, sharing, idea generating, and group actions across community.current priority areas - dementia chorus similar to the giving voice chorus, dementia friends training across the Community, youth outreach, training for restaurant wait staff, support respite care programs and identify gaps in dementia services across northeastern Minnesota.
