Ocfs residential services - empower - a safe harbour programour empower program remains The only program licensed by nys to serve youth in foster care who are at risk of or have been victims of exploitation and/or complex sexual trauma that require residential and supportive services. The program accepts adolescent girls and transgender youth ages 11-21. Common factors include absconding from home/placement and engaging in unhealthy relationships with peers and older adults, including intimate partner violence. Ocfs residential services - supervised independent living (silp)helping our youth move from a place of trauma to finding healing and hope is The number one priority at The Children's Home. In addition, efforts are made to help older youth develop independent living skills. The silp program supports youth who live on their own in The community and provides a transitional experience for youth for whom The plan of care is to discharge to their own responsibility. Youth in this program are between 18 and 21 years of age, have received their high school diploma or ged, and are attending college and/or working in The community. Each apartment is fully equipped so that youth learn meal preparation, housekeeping, financial management, and other skills needed for living independently. Community based services - care management servicesthe Children's Home provides care management services as part of The nys health Home care management program. Children from birth to age 21, who are medicaid eligible, receive assistance from a care manager to help manage The child's chronic and complex medical or behavioral health condition. The goals of these services are care coordination to ensure youth have everything necessary to stay healthy, promote wellness, and decrease utilization of The hospital. Community based services - children and family treatment and support services (cftss)children and family treatment and support services, available with nys Children's medicaid, give children (under age 21) and their families The power to improve their health, well-being, and quality of life. These mental health services include individual, family, and group therapy to strengthen families and help them make informed decisions about their care. Additionally, The services include specific targeted intervention and skill-based supports which will help to prevent The need for emergency room visits, hospital stays, or out-of-home placements, such as group homes or campus-based care. Community based services - wendy's wonderful kids programin 2018, The Children's Home of Poughkeepsie received two grants from The dave thomas foundation for adoption, a national non-profit that brings children waiting in foster care one step closer to being adopted into safe, loving, and permanent homes. The grants are dedicated to funding child-focused adoption recruiters for wendy's wonderful kids (wwk), a signature program of The foundation. Community based services - Home and community based services (hcbs)home and community based services is a strengths-based program that offers services such as respite, caregiver family support, and prevocational skills to youth. Hcbs is certified through The new york state department of health and available to youth who are enrolled in health Home care management. This program provides services and support to families to prevent out-of-home placement for their child. Ocfs residential services - qrtp - agency boarding homes The goal of The qualified residential treatment program (qrtp) is to provide each youth with an opportunity to strengthen their ability for close interpersonal relationships within a Home setting and a neighborhood; to develop skills for independent living and to survive The adolescent identity crisis with people who can provide more intensive help when it is needed. Each off-campus Home has direct care staff who provide daily guidance and supervision. Staff regularly interact with school personnel, transport youth to family visits, and teach independent living skills.
