Vdh (Vermont dept of Health serves as the fiduciary for vpqhc's bill back funding from the hospitals and Health plans in Vermont. With these funds. Vpqhc provides services that are part of the implementation and maintenance of a state-wide Quality assurance system. Through various projects, vpqhc provides on-going evaluation and improvement activities that enhance the Quality of Health Care services rendered in Vermont.
Vpqhc supports Quality improvement activities that improve the clinical and improvement activities that enhance the Quality of Health Care services rendered in Vermont. Quality and performance of all critical access hospitals.
Vpqhc administers the patient safety serveillance improvement system and facilitates root cause analysis, corrective actions plans and aggregates public reporting for serious reportable patient safety events for the state of Vermont.
(d) vpqhc supports the Vermont department of corrections to provide Quality improvement activities. (e) the eastern us Quality improvement collaborative, known as eqic, was established in 2020 to deliver services for the centers for medicare and medicaid services' hospital Quality improvement contract to further advance the Quality and safety of Health Care provided in hospitals across the region. Vpqhc is the subcontractor for this work in Vermont. (f) vpqhc is administering a Program for suicide education and training for independent mental Health providers. (g) vpqhc implements planning, training and technical assistance focused on suicide prevention as part of Vermont's centers for disease control and prevention (cdc). (h) vpqhc received funding to purchase, assemble and distribute youth activity kits to hospital emergency rooms. (i) vpqhc received a grant to support Health equity trainings. (j) vpqhc received funding to support planning including research and stakeholder engagement for future launch of statewide telepsychiatry network. (k) vpqhc performed an external evaluation of client's effectiveness in attaining the goal to create a system of Care coordination that will address the needs of all residents of the region beginning with the highest needs individuals - patients being discharged from regional hospitals."
Vpqhc provided project management services for a project to re-develop the all payers database into a ready analysis database.
Vpqhc supports Quality improvement activities that improve the clinical and improvement activities that enhance the Quality of Health Care services rendered in Vermont. Quality and performance of all critical access hospitals.
Vpqhc is administering a Program for suicide education and training forindependent mental Health providers.
