Integrated Care teams provide comprehensive coordination of primary medical health Care needs, as well as dental, pharmacy and behavioral health services. During 2022, Unity Care Northwest provided services to 21,685 unduplicated patients and a total of 97,333 visits at three clinic sites, with 2,812 visits provided via telehealth. Over 52% of patients served live at or below 100% of the federal poverty line and 83% are at or under 200% of this scale.
