Program areas at Trillium Health

Trillium Health operates two fully-accredited retail pharmacies, designed for convenience and accessibility. Pharmacy services are available to patients at 259 monroe avenue and 170 science parkway in rochester, new york. Prescription services are supported by text reminders, free home delivery, and 24/7 access to medication to help prevent hiv infections. Our pharmacy's average co-pay combined with our financial assistance programs, helps to keep the cost of medication affordable. Approximately 129,905 prescriptions were dispensed in 2022. Trillium Health has both mail order and specialty accreditations for its pharmacy and have contract pharmacy agreements to assist other entities and federally qualified Health centers.
Trillium Health's care management & supportive services programs provide vital services to help break down historical barriers to Health care, such as transportation, low Health literacy, lack of insurance, housing instability, and food insecurity. In 2022, over 8,188 care management services were delivered, providing much-needed support for our patients.
Trillium Health's community-based clinics deliver high quality and affordable care for all, including lgbtq and underserved populations throughout the region. Services include primary care for adults and children; infectious disease testing and treatment; and gynecological care. We served more than 10,912 patients in 2022, (not including patients that were seen for covid-19 testing or vaccinations). We continue to work to increase linkages to care among individuals representing communities of color-from 2018-2022, we've experienced a 107% increase in our black/african-american patients. Our transgender center of excellence has grown 89% since 2018. Our care delivery process continues to remain nimble to the changing healthcare environment, resulting in shorter patient appointment times, more effective utilization of patient care team members, improved clinical workflow, and daily interdisciplinary cross-department communication.
