To provide assistance to organization's that provide support, services and guidance to individuals and their families affected by Alzheimer's and dementia.
Case Management, Education and Awareness: The RSFA provides interventions, support, guidance and referrals to individuals and their families affected by Alzheimer's and dementia.The goal of the program is to offer services to enable seniors to remain at home by creating a safe environment through evaluations, home visits, customized safety plans and care coordination with community resources.In addition, the RSFA offers education and guidance through support group meetings and educational workshops. The support groups are intended for caregivers, loved ones and friends of those suffering from Alzheimer's disease. The meetings are free and open to the local community. The educational workshops are targeted not only towards family members and caregivers, but also towards professionals in the community. The purpose of the program is to increase awareness about Alzheimer's and dementia and to educate professionals.
