Community health workers - The Foundation provided specialized community support services respecting The culturally diverse communities it serves. These include programs in underserved communities, programs for veterans and for religious communities.
Patient assistance and charity care - The Foundation provided these grants to cover The cost of hospice care for individuals who are uninsured or underinsured to The extent that they do not have The ability to pay for hospice care privately. These funds were also used for comfort and joy requests to assist patients and families by improving quality of life and fulfilling end of life goals.
Work force development program - initative to support education, recruitment and retention of three roles; certified nursing assistant (cna), registered nurse (rn), and community health worker (chw) through formal apprenticeship programs in partnership with community colleges across The region.
