Transitions is a short-term residential program providing therapeutic interventions for boys 13 to 17 years of age. The transitions program is designed to comprehensively assess residents' treatment needs to successfully eliminate barriers that prevent healthy family dynamics with family, adults, and peers.the program is supervised by a clinical services manager who directs and coordinates individualized treatment planning. Life skills specialist, recreation specialist, and child care specialists offer critical skill-building and resident supervision. An individualized treatment plan is developed and implemented utilizing an array of services in partnership and coordination with community resources.
