Crisis stabilization Tellurian currently operates care center facilities in madison (16 bed) and la crosse (10 bed) cbrf's (community based residential facilities) under dhs 83. Tellurian's care centers provide a safe haven for individuals who are experiencing a mental health crisis and are in need of support, stabilization, medication observation, and goal orientated treatment planning. The length of stay for each consumer is dependent on the needs of the individual and typically is 7 days or less, with the average length of stay lasting 5-7 days.
Adult residential services - treatment includes individual and group therapy, and specialized counseling for forgiveness and grief. The Tellurian residential program specializes in substance use disorders with an emphasis on co-occurring mental health disorders. Many patients struggling with substance use disorders have accompanying mental health disorders, such as depression, anxiety, or post-traumatic stress disorders. The Tellurian medical director, a specialist in addiction medicine and member of the american society of addiction medicine (asam), provides medical oversight, including health screens and diagnosis of underlying health issues. Psychiatric care is provided as appropriate or recommended, assuring that any co-occurring mental health disorders are identified and treated simultaneously. Medication assisted treatment is provided at this facility.
Housing - Tellurian's transitional housing program (thp) is a 20 bed facility which supports individuals residing within the program who are experiencing homelessness and need support with finding housing, employment, and mental health and/or substance use support. Acewood and crawford are each 7 bed, fully licensed, class a community based residential facilities which serve individuals with a primary diagnosis of mental illness who have the ultimate goal of living independently after learning the tools, skills, and receiving motivation and encouragement through the staff. Hud programs provide up to 37 units of permanent supportive housing, single room occupancy to people coming from homelessness, living with a mental health diagnosed disability, and are dane county residents. These programs provide housing case managers who work closely with landlords to help maintain housing as well as provide support with attending appointments, taking care of basic needs, and providing a harm reduction approach to changing or reducing high risk behaviors. In addition, Tellurian's sober living homes provide housing for individuals transitioning back into the community after successfully completing residential substance use disorder treatment. Jeremy's place for men and chandra's house of hope for women offer additional support and wrap-around services such as mentorship from resident house managers, outpatient treatment services through Tellurian or partner providers, and access to other needed community services to assist with long-term recovery.
Emergency services - Tellurian's detoxification center is a 29-bed facility staffed by highly trained, credentialed professionals. Patients at the detoxification center are medically monitored by a registered nurse (rn) and supervised by a board certified physician specializing in addiction medicine. As part of the detoxification process and treatment protocol, Wisconsin state certified counselors meet individually with each patient to help determine the optimal treatment level and expedite entry into community treatment programs. The average length of stay for most patients at the Tellurian detoxification center is 72 hours or less. The detoxification center operates 24 hours per day, 365 days per year. Tellurian also provides a treatment readiness program, withdrawal management services, a naltrexone program, assessments and case management.
Case management - community intervention team, cit, provides targeted case management and comprehensive community services (ccs) for individuals living with mental illness and/or substance use. Services include: crisis stabilization, medication management, money management, housing coordination, and activities of daily living (adl) skills training. The goal is to promote recovery and enhance the individual's quality of life by improving their skills, competencies, social integration, independence, and life satisfaction. Tellurian's path team provides outreach working to connect homeless individuals in the community with needed services. Tellurian's rethke program provides case management services to residents living at rethke terrace, a permanent supportive housing model serving up to 60 residents.this team does street outreach across dane county and assists people with establishing disability, connecting with benefits, and helping to get housing. Charm provides housing navigation services to individuals at risk of losing our without permanent housing. Tellurian has recently expanded its cit team model to the la crosse area and provides psycho-social rehabilitation services including individual skill development and wellness management for individual adults living with mental illness and/or substance use concerns. Cit works with clients on a variety of services based on their ccs recovery plan. These services include 1:1 support and coaching with completing tasks in the community, maintaining independent housing, support in understanding their mental health and learning coping skills.
Outpatient - the Tellurian outpatient treatment program is the least restrictive level of care Tellurian offers to its patients. Tellurian's treatment is highly individualized and based on specific patient needs as assessed by their counselor. Treatment length and schedules are determined by each patient's individual progress in completing a mutually agreed upon treatment plan. The Tellurian outpatient clinic provides both group and individual counseling, including a specific relapse prevention group. The synergy day treatment program consists of four hours of group four days per week. In addition to the daily group sessions, patients of Tellurian's day treatment program also receive weekly one-on-one individual therapy. Family counseling is available and recommended to help loved ones understand the nature of addiction and mental illness. Participants learn essential recovery tools that can enhance family functioning while supporting the patient's continued sobriety and recovery.
Medical psychiatric services - the med psych clinic provides individual initial and ongoing medication checks with a psychiatrist to individuals connected with Tellurian's community intervention team.
