Case management-swirca & More care managers provided some sort of assistance to 6,353 unduplicated clients between 07/01/2021 through 06/30/2022. The types of assistance provided by a care manager may include eligibility determinations for nursing home placement, eligibility determinations for home and community based services, general advocacy, resource linkage, and/or on-going monitoring of clients receiving funding for in-home services through any of Swirca's funding sources. on the latter point regarding individuals who received in-home services, of the 3,531 individuals who received assistance from a care manager, 2,362 individuals received direct in-home assistance from at least one of Swirca's five funding sources (ad waiver, choice, ssbg, title iii-e, or title iii). The types of in-home services provided through these payers include, but are not limited to, case management, attendant care, homemaker assistance, adult day services, assisted living, home health services, durable medical equipment or supplies, handyman, home delivered meals, or respite services
Wellness and community programs: Swirca & More healthy aging/wellness program serves an average of 3122 people per month (duplicated) with an average of 153.85 per day (duplicated) visiting the activity center. There are a variety of programs offered which include the following: yoga, chair yoga, tai chi, arthritis exercise class, stretch and tone, line dancing, educational health classes (including health volunteers from student nurses, physical and occupational therapists from usi and ue), exercise room, community healthy Aging speakers, social activities (special events/parties, bingo, cards and games, crafts, movie days, volunteer activities, book club, community outings).
Nutrition-the Swirca & More nutrition programs provided daily home delivered meals throughout southern Indiana countries by way of meal routes. The meal recipients include clients who are homebound and unable to prepare a hot nutritious lunch as well as care givers of elderly family members who are unable to travel to a Swirca & More congregate meal site supporting our community members by providing daily well-balanced meals for those in need.
