Case management: in 2024, we successfully housed 51 households through our intensive case management program, providing not just shelter but also hope, stability, and dignity to numerous individuals and families.outreach services: our dedicated Street outreach efforts continued to meet people where they are, breaking down barriers to essential services and ensuring that every individual feels seen, valued, and not forgotten. We supported 6,730 people experiencing homelessness last year and connected 2,994 of them to supportive services.community engagement: through our monthly volunteer drive, we have fostered engagement between community members and the crisis of homelessness, challenging stereotypes that perpetuate homelessness while offering immediate relief to those in need. 430 volunteers joined us in 2024, allowing us to support hundreds of people living unsheltered in chicago.
