Joseph Community partnership Fund organizational commitment believes we have a social responsibility and a moral obligation to respond to the needs of low-income families and individuals served by our ministries. In 1986, St. Joseph Health (sjh) created a plan and began an effort to further its commitment to neighbors in need. With a vision of reaching beyond the walls of its healthcare facilities and transcending traditional efforts of providing free care for those in need of acute care services, sjh created the St. Joseph Community partnership Fund (the Fund) to improve the lives of low-income individuals residing in its local communities. The Fund has contributed over $350 million toward programs that address the Health and well-being of low-income individuals and families in areas surrounding the hospitals. Through the Fund's strategic grant making programs, significant outcomes have been achieved in Community Health settings and clinics, nonprofit organizations, and local sjh hospital ministries.the St. Joseph Community partnership Fund has a number of funding initiatives across four key areas: disaster response and resilience, lifelong education that opens doors, positive change through Community power and Community healing and resilience. Positive change through Community power the Community building initiativethe Community building initiative (cbi) was created in 2000 as a major commitment to meeting the Fund's mission. The Fund's desired long-term vision for its Community building initiative is the development of resident-based capacity to determine and implement positive change in disenfranchised and marginalized communities. This capacity is developed through a coaching model to build neighborhood's capacity to act on their own behalf to address upstream conditions that perpetuate Health, social, and economic disparities. Through cbi, the Fund forged with many individuals whose work exemplified the initiative's vision for distressed neighborhoods to develop_ and exercise their power to make change. Intersections initiativethe Fund have been working to explore a path for advancing upstream, Health equity-driven efforts across providence-st. Joseph Health hospital partners. Since 2017, the intersections initiative has supported local partnerships in seven California communities. Through intersections, partners build capacity within organizations, coalitions, and among decision-makers to advance upstream solutions to Health and wellness in their communities. The intersections initiative plans to expand its serve acrea to invest in regional nonprofits and collaboratives, most specifically in communities impacted by gentrification and displacement. The approach will include engagement with topical experts in the areas of built environment and land use, economic opportunity, safe and welcoming communities, Health equity and racial justice to understand the root cause of poor Health and trauma in communities. Disaster response and resilience initiative the St. Joseph Community partnership Fund understands that human-caused, environmental and natural disasters pose a significant threat to the livelihood of communities, especially those disproportionately impacted by structural and institutional racism. Therefore, our focus is on the long-term recovery of the physical, mental, spiritual, and emotional Health of poor and marginalized communities that often suffer$ the most following a disaster. Recognizing the rising frequency and severity of disasters, the Fund pivoted to provide trusted disaster relief nonprofits with unrestricted grants to enable them to respond swiftly, innovate solutions, leverage resources, and sustain a presence in vulnerable communities. The Fund is positioning itself as a primary funder during emergencies and ensuring timely assistance for affected communities. Education initiative the education initiative is a targeted investment strategy with focus areas centered on the school-to-prison pipeline, prenatal to three, and Community wellness, safety, and liberation education. The Fund will scale the work centered on capacity building so communities can sustain transformative movements. Community healing and resilience initiative the Community healing and resilience initiative is focused on collective healing within Community as an essential precondition towards forging sustainable and meaningful power building. Through partnerships with various community-based organizations, we will explore and learn about the impact of internal healing, organizational healing, and Community healing that contributes to the ecosystem of the social impact field and how harm is manifested within the nonprofit industrial complex.
