Aging block grantthe Southwestern pa Area Agency on Aging, Inc. Serves as a focal point in fayette, greene and Washington counties for information/assistance about Aging issues and services, protecting older adults who are at risk, assisting families who are caring for older family members, supporting and empowering older residents to remain independent and active for as long as possible in their own home and community. Our mission is to promote the well-being of older adults through a planned, coordinated and comprehensive service delivery system.

Aging waiver/chc waiverstarting january 1, 2018, community healthchoices participants now receive their medicaid coverage for physical health services and their long term services and supports from one of the three chc managed care plans. The Southwestern pa Area Agency on Aging, Inc. Entered into continuity of care agreements to provide service coordination with all three of the chc mco's. The Agency assisted participants in obtaining access to needed health services and in-home supports, as well as social and housing services needed to help participants live well in their communities through the development of a person-centered plan utilizing cost-effective long-term services and supports.
