Program areas at Sojourner Fund for Women and Children
Sojourner provides several programs including support services principally to move women towards self-sufficiency and into permanent housing. Sojourner previously provided Transitional, Permanent and Supportive housing in the Rochester area. Sojourner does not provide housing any longer. For the long-term objective, Sojourner has signed an agreement that transferred its existing programs and assets to the Young Women's Christian Association of Rochester and Monroe County in the hopes of running the programs with greater administrative efficiency as well as serving more clients. The Young Women's Christian Association of Rochester and Monroe County also provides housing.The housing and services provide shelter, structure, and case management services to homeless women, and homeless women with children. (Continued on Schedule O)-Multiple health services tabling on-site to assist with connections to Molina, Elite, Excellus,and His Branches.-Telemed services through His Branches began in March 2024, and on-site peer counseling.-Flacra is set up with counselors coming in to meet residents on their floors and in apartments. This began in 2024.-Partnership with the Jazz network in addressing needs of 60+ teen girls in all aspects of case management and support to thrive. This included 1x a week before school groups designed to support teens that have expressed needs and are in danger of suspension. The YWCA youth case manager was involved beginning in December 2023.-YWCA housing case management assistance and support of our existing housing case managers.-Resource management in support of providing easier access to meet the needs of all our YWCA clients and staff in resources and community connection, benefits, and health services.-Trauma Informed care training for organizations via our NYS trained Equity and development staff.-Emergency assistance to support our Emergency Housing team and clients, including courtesy nights for women and children in our emergency shelter.
