Sevamp encompasses the overall management and supervisory functions of the area agency on aging, as well as the lead role for providing a coordinated comprehensive system for older persons to locate and access needed Services. Senior Services' mission is to provide seniors and their caregivers with access to programs and Services, so they may live their lives with choice and dignity in their communities. Supportive Services funded by title iii-b of the older americans act:provided the following Services: legal Services for 54 individuals for 159 hours; provided transportation to 705 individuals for 23,477 one way trips, information assistance and referrals to 2,068 individuals for 9,173 contacts, provided long term care coordinating and planning activities for 3,078 hours, provided public information and education for 223 events, provided socialization events for 251 individuals for 13,437 hours of activity, and we helped 123 individuals with emergency needs.
Congregate meals c-1provided 59,532 meals to 1,175 individuals in a social congregate setting.
Home delivered meals funded by title c-2 of the older americans actprovided 216,819 meals delivered to the home of 2,037 individuals.
Health promotion provided the following Services; provided care transition from hospital to home for 489 individuals with 1,292 contacts; provided options counseling for 557 individuals for 1,343 hours; provided assistance for 6 individuals concerning elder abuse; made 2,673 contacts with individuals concerning their medicare benefit; 205 individuals participated in 1,964 evidenced based wellness classes and provided 29 people with nutrition counseling.
Caregiver Services - title iiiethe caregiver Program helped seniors and their caregivers receive a variety of Services. The Services include: care coordination for 47 seniors for 231 hours; provided adult day care to 48 seniors for 18,896 hours; provided transportation to 7 individuals for 359 one way trips to day care Services, information assistance and referrals to 500 caregivers for 1,341 contacts: provided 2,797 hours of homemaker Services to 17 caregivers and provided 136 public information and education sessions for 1,731 caregivers.
All othersother Program service areas include 68 Senior companions served 154 seniors with 48,093 hours of Services, we provided 38 people with 2,610 hours of in home homemaker Services; we provided 64 individuals with 2,790 hours of in home personal care
