The program goals are to prevent hospital or facility readmission, facilitate recovery, and defer nursing home placement, 162 unduplicated homebound seniors received emergency meals on wheels, 2,281 meals served.

Direct elderly care adult day care - provides coordinated health, social, and therapeutic activities in a supervised group setting for adults with functional and/or cognitive impairments. 131 unduplicated clients served; 100% deferred from nursing home placement.
