This program is for individuals needing a higher level of care coordination and supportive living, PCMS manages assisted living programs that allow clients to be cared for in a nurturing family environment. Individualized care plans help clients work toward reaching as high a level of independent living as possible.
Journey Life Care Program is a health and human services care management program that is comprised of professionals whom serve as a navigator. The professional geriatric care management services are a holistic, client-centered approach to caring for aging adults or disabled persons. The goal is to improve the client's quality of life and reduce family stress through providing a care plan that changes as the needs of the client needs.
