Care coordination: the organization completed 2,966 holistic comprehensive assessments for older adults, their families and caregivers during fy22. Based on the identified needs of the older adults, orgainzation staff made the appropriate referrals to state and federally subsidzied programming to assist the older adult to age in place safely in the community for as long as possible.
Adult protective Services: during fy22 the organization investigated 495 reports of abuse, neglect or exploitation of an older adult or an adult aged 18-59 yers old with a disability. Case workers conduct investigations and work with in resolving the abuse, neglect, or financial exploitation situation
Title iii-c2: home delivered meals are provided to individuals over 60 years of age who are either temporarilty or permanently unable to manage meal preparation and access food. The nutritious home delivered meal service doubles as a well being check. This is critical for individuals experiencing social isolation. During fy22, the organization delivered a combined 38,385 meals, hot and frozen. Additionally the organization coordinated delivery of 13,017 special diet meals and 2,450 shelf stable meals.
