Home delivered meals - the Pinal-Gila Council for Senior Citizens (pgcsc) home delivered meals programs provide a nutritious meal to seniors and persons with disabilities to help increase their nutrient intake, lower their nutritional risks and allow them to remain independent in their homes. A daily nutritious meal, containing 1/3 of the recommended daily allowance is prepared and delivered to participants through 1 of 9 contracted services providers and directly through pgcsc. In addition to the meal, program participants also receive a wellness check to ensure their general health and wellbeing, and receive important information and resources that allow them to remain in their homes. During fiscal year 2014/2015 pgcsc served over 132,000 meals to 1,201 individuals residing in the Arizona counties of pinal and gila.
Case management - all pgcsc home-care and home delivered meal services are authorized and monitored through structured case management systems. Case managers identify, facilitate, and coordinate formal and informal services in order to assist clients to maintain independence and avoid institutional placement, while complementing caregiver support. This is achieved through initial home visits to assess the client's strengths, needs and eligibility , and periodic telephonic reviews to monitor serv
Home care services - pgcsc offers a variety of coordinated in-home care and support services for seniors and persons with disabilities, in their place of residence, to help them maintain their independence and avoid costly and unwanted placement in a care facility. Included in these programs is housekeeping services, which provide assistance with performing activities related to routine household maintenance in order to maintain and improve safety and sanitary living conditions.
