Program areas at Pathways

Residential services - agency residential program options offer a variety of 24-hour supervised home settings for children with severe emotional and behavioral difficulties and for adults with developmental disabilities including 5 individual residential alternative (ira) homes, 3 intermediate care facility (icf) residences, and 2 community residences for children and youth ages 5-17.the agency also serves youth in custody of the department of social services (dss) who have serious emotional or behavioral difficulties and/or developmental disabilities in need of a therapeutic out-of-home placement. Through placement with a resource (foster) parent, the program provides a local home to keep children and youth in their community. Steuben county dept. Of social services collaborative programs serve children who are at imminent risk of out-of-home placement and their families and require referral from steuben county dss.
Care coordination and contract management - these services provide care coordination to youth and adults with chronic medical conditions to get the care they need to improve their health and supports. Additionally, service coordination for individuals that are nursing home-eligible or have traumatic brain injuries. Pathways Inc. also provides contracted services with the steuben department of social services for at-risk youth and families, steuben county community mental health services for adults and children, chemung county department of social services for parent advocacy, and steuben county public health.
Community and habilitation services - these programs provide various day programming and employment options for adults with intellectual and developmental disabilities, and support for individuals that are nursing home-eligible or have traumatic brain injuries, in addition to behavioral health services for adults with mental illness and/or alcohol and substance abuse. Such services include day habilitation, community habilitation and employment services.additionally, programs include support services for children with serious emotional difficulties, developmental disabilities (if in foster care), and medical fragility and their families.
Childcare services and clinical services - these programs offer daytime care and educational programming from infant through school-age and before and after school care at corning-painted post elementary schools.through june 2022, the agency offered special class preschool services for developmentally delayed children with speech/language therapy, physical or occupational therapy, sign language, and hearing impairment in steuben and chemung counties.
