Covid response project - providing covid-19 test to the community and travelers for the city & county of honolulu.
Public health education - all residents are welcome to visit our website at www.kidneyhi.org to learn about Kidney disease and the wide array of programs and services available, including presentations, screenings, self-management education, cookbooks, youth programs, patient support groups and clinical services such as care management nutrition counseling and social services. Programs are interactive and encourage healthy lifestyle choices.
Patient services - nkfh provides direct services to thousands of individuals each year, especially those with Kidney failure. We serve patients on dialysis or with a transplant and their families and caregivers. Nkfh is a major advocacy champion for patients and has established councils and support groups as a forum for their interaction. Nkfh also provides patients with much needed funds for emergency needs, medical id jewelry and handbooks. Nkfh will match up patient mentors with those newly diagnosed, to enhance their quality of life. Our most significant effort is to prevent Kidney disease and its progression in the first place.
Community service - promoting awareness of Kidney disease to enhance lives through education through our participation in health fair, community events, company wellness programs and presentations.
Research - initiative to focus on tracking the outcomes and impact of our services and building a chronic Kidney disease registry to improve health outcomes and transform chronic disease management in Hawaii in order to effectively respond to chronic disease by population-based reporting the chronic disease burden (surveillance). The results will provide information that will decrease healthcare costs and improve outcomes through increased early identification, multidisciplinary interventions, evidence-based education for patients and healthcare professions and scientific research.
