Program areas at Mount Carmel Guild of Trenton NJ
The community support program continues to care for the homeless, hungry and food insecure in mercer county. Since april 2019, the food pantry operates jointly with catholic charities diocese of Trenton at the Guild's campus at 73 n. clinton ave., Trenton. In response to the covid-19 pandemic in march, households began using the pantry as needed instead of once a month. The pantry served an average of 430 single household visits and 471 family households visits each month, impacting 23,037 care receivers in the fiscal year. Additionally, 45 senior households are enrolled in the monthly usda commodity supplemental food program. Our thanksgiving dinner package provided a complete meal for 659 families and over 175 families received toys and gifts for christmas. Holiday projects and food collections would not be possible without generous support from local parishes, schools, corporations, and civic organizations.
The home health nursing program is accredited by the national institute for home care accreditation, yet services remain free to low-income elderly patients throughout mercer county. The Guild does not receive reimbursement from medicare or other insurance. This year 51 patients received a total of 1,838 units of service (up to 45 minutes a unit for direct care). Registered nurses provided comprehensive assessments, vital signs monitoring, medication management, prefilled syringes, injections, glucose blood checks and blood thinners monitoring. Twenty percent of patients have been in the program for over 4 years. A total of 66% of patients remained medically stable and living in their own homes at year end, aging with grace and dignity. All patients who required additional social services were linked to needed services.
