Mobile Integrative Navigation Team

Navigation Center: Provide medical care and support to more than 500 individuals with urgent health needs. Examples of the services provided are wound care driving clients to medical appointments making hospital visits picking up medications transporting clients to alcohol and drug treatment and coordinating care with other service providers. Coordinated 52 days of weekly outreach efforts distributing essential resources such as food shelter and water to those in need. Successfully assisted over 30 individuals in securing stable housing thereby helping them transition out of houselessness.

Inclement weather center: delivered both winter and summer shelter to over 1,000 non-unique individuals facing adverse weather conditions due to their homelessness. Each day or night that we were open guests signed in and we were able to serve 40-90 unique individuals during each 24 hour period. .
