Program areas at Merced Rescue Mission Support Corporation

Merced Rescue Mission operates a New Life Transformation Program called "Hope for Men," "Hope for Women, and Transformation to Hope". These are 9-12 month faith-based programs. These programs receive men and women who have been incarnated, experiencing homelessness, struggling with addiction issues or simply feel hopeless. These programs address addiction issues, and build life skills that help people become self-sustaining through finding jobs. Family reunifications also an important part of this program. Mission Merced operates the Navigation Center which is Emergency Homeless Services for people living on the streets (daily meals, showers, clothing, shelter, etc.). This program is a service-rich environment where Golden Valley Health Centers operates a clinic, Behavioral Health Department provides a staff person and the Human Service Agency provides a staff person. The Navigation Center is also staffed with a full-time Housing Navigator and Case Managers. This is a co-ed and pet-friendly environment. The goal of this program is to help people find permanent housing. The Bridge to Hope Program (5 homes) is similar to the Navigation Center, where 6 to 8 people share a house with a house manager. The goal of this program is to get people into permanent supportive housing and to help people find job while becoming independent. The Hope Respite Care Program Individuals experiencing homelessness who are discharged from the hospital into the Hope Respite Care program, receive whole-person care coordination while they recover. This innovative collaboration includes relationships with Mercy Medical Center Merced, Sutter Memorial Hospital Los Banos, Central California Alliance for Health, Golden Valley Health Centers, and Stanislaus State University Our Hope Respite Care locations are friendly, welcoming and offer participants access to the full suite of comprehensive health care services as well as case managers, transportation services, mail pick up, housing navigation and access to supportive services. We have entered into a relationship with Central California Alliance for Health to provide Enhanced Care Management and Community Supports for our clients on Medi-Cal. Enhanced Care Management (ECM), helps guide clients with their medical care. ECM can also help them find community services that are related to their care. Community Support helps with housing, getting food after a hospital discharge, hospital discharge support and more.
