Program areas at Lone Star Family Health Center
Community Health Center (chc) - as a chc, we provide full spectrum primary care services including Family medicine, obstetrics, gynecology, general dentistry, behavioral Health counseling, diabetes, nutrition counseling, x-ray, laboratory services, mammography, pulmonary function testing, stress testing, pharmacy services, and other outpatient procedures. Clients that report income at or below 200% of the federal poverty level are offered service discounts on a sliding scale. In 2022, we provided over 100,000 outpatient visits to more than 34,000 unique patients and extended more than $1.2 million in sliding fee discounts directly to patients.
Community centered Health home program - Lone Star participated in social determinant of Health work that involved partnering with local schools to develop programs around childhood obesity.mental Health innovation program - Lone Star also worked on a program to collaborate with our local school district and the mental Health authority to coordinate mental Health services between organizations.
Conroe Family medicine residency program (cfmr) - we operate the conroe Family medicine residency program that trains 32 Family medicine residents over a 3-year period in partnership with the conroe medical education foundation (cmef) and hca houston healthcare conroe. Cfmr is an unopposed, acgme-accredited, community-based residency with a long history of preparing highly trained physicians who practice in a variety of settings. In 2022, 100% of graduating residents passed their medical board examinations.
