Covid disparities grant expands lsa's reach into the community and connects more people to critical services. the community Health workers (chw) in this program offer educational workshops on various Health and community resource topics, offer navigational services to connect individuals to resources that they need, and provide Family support services to ensure that resources are accessed. Additionally, chws provide community education around the benefits of vaccation against covid-19 and connect program participants to resources to obtain vaccinations in one of the hardest hit areas in new york city.
Our certified home Health agency provides full-scope nursing care to a population comprised of first-time mothers, high-risk maternity patients, infants, pediatric patients and the elderly. Our visiting nurses practice holistic, patient centered care in the home setting and advocation for those we serve. Our partners at local hospitals and neighborhood clinics rely on lsa's nurses to deliver care to their patients with skill, compassion and warmth through the infant mortality reduction initiative, our nurses have helped reduce the disproportionately high rate of infant deaths in east harlem,
the advocacy and food pantry programs help families meet their need for food and addresses longer-term issues of living in poverty. Our advocates educate families about their rights and empower them to access needed services and resources. We assist families with public benefits applications (including snap enrollment), refer them to lsa and non-lsa programs, provide connection to immigration legal appointments and help tenants avoid eviction and get needed repairs to maintain a safe home. Visitors to our food pantry receive fresh and healthy food choices after meeting with an lsa advocate who evaluates the full extent of their needs.
