Glade House - short-term housing (single room occupancy- sro). Residential House with 10 beds. Provides short term housing for up to two years. Services provided include intensive case management, mental health counseling, monthly tarc passes, life skills education, individual and group counseling. 12 clients were served, 6 exited to long term housing, one exited to a substance abuse program and 5 remain in sro. Bed utilization rate is 67%. Program revenue - 28,170 program expense - 209,789
Hwh long term housing - supportive housing in 13 scattered-site homes with deed restrictions and no mortgage. These House hiv positive people whose lives are further complicated by homelessness. 19 households with 23 adults and 9 children were housed and received subsidized rent, case management and emergency financial assistance. The unit utilization rate was 87%. Program revenue - 15,614 program expense - 169,878
Other forms of financial assistance: childrens program we provide back-to-school assistance including clothing vouchers, backpacks, and school supplies. Case management we provide treatment planning where clients set their own concrete, achievable goals to address mental, physical, emotional, social, financial, educational, and employment needs. Financial assistance tarc tickets, rental security deposits, furniture, rent and utility payments, moving expenses, uniforms needed for work or school, and funeral expenses
