CARE IS PROVIDED WHEREVER THE PATIENTS LIVE; IN THEIR HOME, IN A HOSPITAL, ASSISTED LIVING/LONG-TERM CARE FACILITY OR at two HOSPICE HOUSE residential facilities. IN 2018, HOSPICE OF WESTERN RESERVE PROVIDED 411,165 PATIENT DAYS. CARE IS PROVIDED BY A TEAM OF PAID AND VOLUNTEERS WHO ARE AVAILABLE 24-HOURS A-DAY, SEVEN DAYS A WEEK. ADDITIONALLY, BEREAVEMENT SERVICES ARE PROVIDED FOR FAMILIES FOR MORE THAN A YEAR FOLLOWING THE LOSS OF A LOVED ONE.
