We continue to expand our services adding care management to our list of resources in order to help our patients improve their health and quality of life. We currently provide primary care, some specialty care, labs, medications, counseling, and care management. We also do social assistance referrals for those in need of food, shelter, utility support, job help, dental, vision or other care resources.
