Administration for community living - provides information and referral, peer support, advocacy, transition support, and independent living skills training to people with disabilities.
Support coordinator - assists ddd eligible individuals in accessing community-based supports and services to enhance their independent living by identifying appropriate programs and providers, facilitating socialization skills, and building life experiences with the ultimate goal of securing employment.
Support for independent living - provides linkages to community-based programs for people with disabilities as well as financial assistance for the purchase of adaptive equipment and home modifications that remove architectural barriers in their homes.
