: In 2023 Hearts For The Invisible Charlotte Coalition served 238 individuals in Mecklenburg County. Approximately, 48% identified as a woman, 50% identified as a man and 2% identified as LGBTQ-IA. Additionally, HFTICC served 115 people who reported mental health issues, 96 people who reported chronic health condition, 62 with a physical disability and 28 with a developmental disability. Our team provided benefit assistance with 67 individuals, 40 healthcare referrals, 66 shelter referrals, assisted 77 individuals with housing search and applications, connected 45 individuals to mental health services, 20 individuals to substance use services, and helped 89 individuals obtain their vital records. Lastly, HFTICC's Outreach team achieved 107 positive exits from our homeless street outreach program which resulted in 57 households going into the shelter, 10 into transitional housing situations, and 35 individuals into permanent housing.
