Imad personal responsibility education program (prep) in september 2015, Heartland Rural Health Network was awarded a personal responsibility education program grant to conduct comprehensive Health education programs in highlands county using approved, evidence-based curriculum, Health educators from hrhn were able to provide education to youth ages 10-19 in highlands county.rural Health care services outreach grant (rog) program hardee desoto community Health worker program (hd chw program) - hrhn was awarded the hrsa Rural Health care services outreach grant in april of 2021 to develop and implement the hardee desoto community Health worker program (hd chw program). This program will improve access to anad navigation through care for the minority population, especially the hispanic/latinx, a subset of the minority population, of Rural hardee and desoto counties, fl. The hd chw program os a care coordination program headed by a consortium of community Health leaders that aims to assess the clinical and non-clinical needs of patients, provide appropriate Health pathways through warm hand-offs and partner referrals, assistance when patients are faced with issues they are incapable of handling on their own, as well as follow-ups to ensure needs and appropriate preventative diagnositics are continuing to be met and providing needed Health education. This consortium is comprised of leading Health organizations located in hardee and desoto counties. This program will provide community Health workers as care coordinators, an evidence-based model, and utilize customized case-management plans, pathways to care, warm hand-offs for referrals, and motivational interviewing to enhance their impact in reaching the program's goal of empowering and supporting the target population to improve access to care while making it easier for the target population to navigate the appropriate channels to receive all needed and requested care, improving their overall impression of their current Health status.
