Grant Medical Center

Provided medical services to underserved population in rural west virginia and bordering Kentucky and Ohio counties under a community health centers grant from the U.S. Dept. of Health and Human Services. Other services provided to patients include: Nutritional, Diabetes education, social determinant services, patient advocacy services, transitional care management, chronic care management regardless of a patient's ability to pay. Additionally, financial assistance to eligible patients for laboratory and diagnostic testing services and pharmaceuticals. Occasional screenings for cholesterol, along with free blood pressure checks, are also provided. Provided medical laboratory testing services. Also provided $693,233 in charity care to indigent patients.
