Concordia Physician Practice

Concordia Physician Practice provides medical services to the residents of Concordia Lutheran Health and Human Care's (EIN 25-0969458) independent living, personal care, and skilled nursing and rehabilitation facilities as well as the surrounding community. The organization provides appropriate care to patients that allows them to stay in the least restrictive, least costly level of care in the Concordia continuum or within their homes as applicable, as well as allowing for at home visits to limit hospital stays for patients. The organization provides care the patients who are covered by most major insurance plans commonly accepted in the region, including Medicare and Pennsylvania Medical Assistance for the indigent.
