Program areas at Community Home Health Services
Home care - for more than 50 years, our staff of registered nurses, therapists, and medical social workers have provided patient care in the comfort of the patient's Home. In 2019, we provided Home Health Services to 5,223 patients, 33,472 skilled nursing visits, 36,700 physical therapy visits, 13,435 occupational therapy visits, 6,168 Home Health aid visits, 2,247 speech therapy visits, and 731 medical social worker visits. Our staff is experienced and professional with advanced, specialized education and certifications in order to provide the patient with excellent quality care. In addition to skilled nursing care, we provide several specialized nursing Services including life's journey - palliative care, cardiac care, wound care, and tele-health monitoring. We also provide Home rehabilitation Services including physical therapy, occupational therapy, and speech therapy. We strive to provide excellent care in the Home and continue to look at ways to improve and new ways to deliver care to patients.
Hospice - hospice care is for patients that have an illness or condition with a life expectancy of six months or less and an absence of curative treatment. Our staff are dedicated to provide high quality of life at the end stages and will help support the patient and the family in meeting their physical, emotional, and spiritual needs. Through our staff of registered nurses, Home Health aides, medical social workers, therapists, and chaplains, in 2019, we provided care to 1,160 patients and approximately 2,603 bereavement patients. We had a total of 34,066 days that included 1,077 inpatient days and 387 respite days. In hospice, we provide many Services, including pain control and symptom management, medical director support, registered nursing care, emotional and spiritual counseling, Home Health aide support, volunteer assistance, diet consultation, pharmacy support, and bereavement support for the family.
Home medical equipment & respiratory - Community Home Health Services provides a wide variety of Home medical equipment which enables patients to live comfortably and safely at Home. When our patients receive their equipment, they also receive comprehensive education on the operation, safety, and maintenance of the equipment. The sale of such equipment is to patients of Community Health network. For our respiratory Services, our highly skilled respiratory therapists and staff provide management of sleep disorders and respiratory Services, such as, respiratory and oxygen equipment, inhalation therapy Services, and cpap/bipap sleep disorder programs. In 2019, we had an estimated daily census of 1,454 patients on oxygen and an estimated daily census of 2,375 for cpap.
