Program areas at Community Care Connections
The Community home program provides supported residential Care 24 hours a day, 7 days a week to individuals who have intellectual, autism, and sometimes physical disabilities. Services are provided in ten separate two or three-person homes located in residential neighborhoods throughout butler county. The program employs specially trained staff, many of whom have worked in the program for 15+ years. Individuals enrolled in the program also attend educational, vocational, employement or adult day service programs. During the period 7/1/22-6/30/23 Community Care Connections' Community home program provided residential support services to 30 individuals.
The infant-toddler therapy services program provides in-home therapy and instruction to infants and toddlers, ages birth to three, who have developmental delays. The therapies and instruction offered through the program include speech, physical, occupational, vision and/or hearing therapies, special instruction, and nutrition counseling. Therapies are provided by specially trained and licensed professionals. Parents participate in each therapy session and are taught activities designed to help their children develop to their fullest potential. During the period 7/1/22-6/30/23 Community Care Connections' infant-toddler therapy services program provided 19,758 units of therapy to 190 infants and toddlers in butler, beaver, armstrong, lawrence, and allegheny counties.
The attendant Care service coordination program provides service coordination for adults with physical disabilities and who are mentally alert, who receive attendant Care services. Specially trained service coordinators assist individuals to develop individual service plans that outline what personal Care and housekeeping services they need to remain in their homes and communities, connect with local providers who provide the needed direct personal Care and housekeeping services outlined in their individual service plans and secure assistive technology, adaptive equipment, or home modifications as needed. During the period 7/1/22-6/30/23, Community Care Connections' attendant Care service coordination program provided 3,724 units of service coordination to 27 adults living in butler, beaver, lawrence, mercer, venango, clarion, crawford, armstrong, westmoreland, and allegheny counties.
The stay program provides life skills training to adults who have intellectual, sensory and/or physical disabilities. Life skills training is offered in the areas of meal preparation, reading, writing, money management, personal safety and disaster preparedness, fitness, appropriate use of leisure time and volunteerism. Instruction is provided on an individual and small group basis by trained staff members. During the period 7/1/22-6/30/23 Community Care Connections' adult services program provided 18,559 units of training to 14 adults.the camp apple program is a fully inclusive recreational program for school age children and teens with or without disabilities. The program offers a summer day camp experience during june, july, and august, as well as saturday recreational opportunities throughout the fall and spring. During the period 7/1/22-6/30/23 Community Care Connections' camp apple program provided 9,437 units of summer day camp and recreational activities to 30 individuals. Community supports program is a collection of daytime service options that empower adults with autism and development disabilities achieve their own unique everyday lives. The go center offers peer interaction and socialization. Participants can choose to receive one-on-one supports totally in the Community. In-home and Community supports are offered in the participant's home and Community to build skills to live more independently. Employment services are also offered through the Community supports program. During the period 7/1/22-6/30/23 Community Care Connections' Community supports program provided 144,594 units of Community supports to 61 individuals.the homemaker and chore program provides light housekeeping services to adults whose intellectual and/or physical disabilities prevent them from maintaining a clean, safe living environment. Service is generally provided once a week, for three to four hours per visit. During the period 7/1/22-6/30/23 Community Care Connections' homemaker chore program provided 2,011 units of homemaker support services to 24 individuals.the behavior supports program provides intensive, specific behavior supports to adults in all waivers including the adult autism waiver. During the period 7/1/22-6/30/23 cccs behavior supports program provided 23,673 units to 13 adults.
