Program areas at Coming Home Connection
In-home care in 2022, Coming Home Connection provided in-home caregiving services to over 100 clients through 48 caregivers. Services include post-surgery support, companionship and long-term care aligned to individual care needs.
Equipment lending & donating the durable equipment loan program provided over 400 pieces of equipment, supporting more than 300 families with no-cost loans of equipment such as wheelchairs, walkers, rollators and bedside commodes
Health navigation the Coming Home Connection health navigator worked with 48 clients, creating navigation plans to help them access medicaid and medicaid waiver service, public resources, local food banks, utility assistance and community-based resources like mental health care and housing options.
No-cost caregiving & housekeeping Coming Home Connection launched a pilot program providing no-cost services for individuals without the ability to pay for in-home caregiving and housekeeping.
