The programs provide a trans-disciplinary team of medical, behavioral health, and rehabilitation professionals who provide housing support and client centered services to chronically homeless individuals experiencing serious mental illness and substance addiction. There are no preconditions or requirements for anyone experiencing chronic homelessness and needing housing support or wishing to receive services.
