Program areas at Choice of Ny
The organization has a homeless outreach program designed to contact clients in shelters and on the streets with the goal to transition into community-based housing. In the case management program, case managers meet with clients at least once a month at the client's home,in a community setting or telehealth to work on goals such as independent housing, benefit acquisition and budgeting.
