For over 73 years, Central Coast visiting nurse association or Central Coast Vna & Hospice has been servicing communities across monterey, san benito, santa cruz, and south santa clara counties. Thousands of patients and their families continue to choose Vna for their home health care and Hospice needs each year. Vna delivers compassionate, trusted, team-driven home health, palliative, and Hospice care and community services with an emphasis on individual patient needs and care goals. We respect and honor patients while educating and empowering families. Vna home health care team provides intermittent, skilled, in-home services, including rehabilitation, restoration of independence, and management and stabilization of disease by licensed, clinical professionals. In 2022 Vna service approximately 86,000 patient care visits including palliative care.
Vna community services brings wellness to the community through employee and school education programs and immunization clinics. In 2023, we provided 2723 flu vaccines, and 2966 covid vaccines, primarily in scheduled clinics or in the home settings.
Vna Hospice involves a clinical, team-driven approach to treating the whole patient, including expert in-home medical care, pain management, emotional and spiritual support, and comfort care expressly tailored to meet the needs of individual patients and families facing end-of-life illnesses. In 2023 Vna Hospice provided approximately 40,000 patient visits and 1,850 volunteer hours. Hospice is available 24 hours a day to care for your patients. Our team specializes in maintaining our patient's dignity, wishes and comfort so they can feel at ease wherever they call home. We excel in providing pain management to our patients.
Vna bereavement team offers multiple grief therapy groups across different county geographies and individual, personalized grief counseling to both community members and Hospice families. Our grief counseling is a free community service provided to anyone experiencing loss.
