Created a Community Health Pathway for Public Health Interns at U of M, St Kates, and University of W, La Crosse to learn while spending their internship in a Fire Station working the 911 Call Referrals alongside our care coordinators to have a better understanding of community-based needs before entering a public health workforce.
We are scaling our FD CHA across MN, we have Duluth, Moorhead, Albert Lea as we work in rural MN in the mix. Our CHA is growing we have Minnetonka, Plymouth, Edina, Richfield, Eden Prairie, Hopkins, and our home base St Louis Park all working as we build out our Care Resource Connections Model!
The State of North Dakota's 5 Major Fire Departments are now partnered with us - West Fargo, Fargo, Grand Forks, Bismark and Minot.
