Program areas at Care Connection for Aging Services
Supportive services: Provide transportation to medical appointments, senior center activities, or necessary shopping; options counseling, care management, in-home services, telephone reassurance, friendly visiting and medical alert systems so that people can remain in their own homes; information and connection to a wide variety of services available in the community; and limited legal services.
Special Projects: Provide one-on-one, unbiased counseling to people who need to enroll or compare plans for Medicare, Medicaid, the Federal Health Insurance Marketplace and the Supplemental Nutritional Assistance Program; the extension of case management and in-home services to senior housing; tax preparation for seniors; volunteer opportunities for a variety of agency programs and our communities; extension of home delivered meals, in-home services and case management services through county tax funds. Operate a statewide Senior Medicare Patrol (SMP) that empowers seniors to fine and report Medicare fraud.
Nutrition: Provide hot, nutritious meals in dining rooms weekdays at lunchtime; hot and frozen meals delivered to seniors who are homebound - all to help person avoid isolation, be well nourished, and remain independent.
Disease Prevention, Family Caregiver, and Ombudsman services: Provide a wide range of services that promote physical and mental health and fitness using evidence-based activities; exercise classes, health screenings, social activities; help in self-management of diseases such as diabetes and high blood pressure; medication management; ombudsman services to advocate for people who live in nursing homes and to prevent elder abuse; and family caregiver services for those providing care.
