In 2018, through ccn's collaborative efforts on the original eleven dsrip projects, they have managed to engage over 100,000 medicaid members when you include all the work you've done through the innovation fund program, vbp, the medicaid accelerated exchange (max) series, and now through the cohort management program and the population health and Care management platforms. These efforts have begun to move health outcomes in our region. In addition, in 2018 ccn started the implementation of population health and Care management software to offer to Network partners.
