Social programs: Capitol Hill Village offers a range of social programming, and includes social features in much of our educational and health-related programming, as a way to mitigate against the harm of social isolation on the health of older adults. An extensive menu of programs includes happy hours, discussion groups, shared meals, and more.light care support services: Capitol Hill Village has a care management team led by a licensed social worker to support chv members with health changes and other personal needs. Light care services includes short-term consultation and assistance, such as home visits to assess safety and general well-being, assistance with accessing groceries and/or prescriptions, developing a plan for volunteer support through the Village, pre-hospitalization consultation for scheduled surgeries, etc.transportation: chv is committed to ensuring that lack of transportation does not present a barrier to accessing health and wellness activities and supports. Through our volunteer driver program, lyft concierge service, and transportation training and advocacy, we ensure our community has access to the information and support they need to make use of available transportation options.
