Program areas at APC
VHC Pediatrics is recognized as a highest level patient-centered medical home by the national committee on quality assurance (NCQA). This recognition is available for systematic use of patient-centered, coordinated care management practices. In 2022, VHC Pediatrics enrolled 746 new children. 4,593 children were evaluated and treated through 17,478 clinical encounters; 47% were acute care visits and 53% were well child visits. There were 1,262 visits for children under the age of 1 month. VHC Pediatrics administered 12,827 vaccines to protect children and the community against life threatening, preventable diseases. Immunizations for 2 year-olds was 92%. VHC Pediatrics provided resources that helped parents obtain or renew 362 Medicaid/FAMIS benefits for children ensuring them greater access to pediatric specialists, hospitalizations, and medications. VHC Pediatrics provided 157 free prescriptions and 1900 free over-the-counter medications to uninsured children to help improve compliance with treatment plans. There were 412 patients identified as children with special health care needs (CSHCN). These children are profoundly affected by conditions such as Cerebral Palsy, Developmental Delay, Congenital Heart Conditions, Asthma, and Down syndrome. Their treatment plans require extensive care coordination. VHC Pediatrics participates in reach out and read and every child is encouraged to take home a book after every visit.
